I got a note recently from a parent looking for an easy data recording system that she could pass on to her child’s teacher. This format is not a new one nor is it one that I designed. My forms are based on those created by Alison Ford and the Wisconsin School Inclusion Project back in the 1990s. So, I can’t take credit for this handy little tool, but I have found it really useful in helping all educators (e.g., general educators, therapists, paraprofessionals) understand how to integrate IEP objectives into general education classes. By assigning objectives to segments of the day, teachers can also simplify data collection.
Data might be collected daily or weekly (depending on student objectives) using this format. Any educator on the student’s team can add comments and student work can even be attached to illustrate challenges, growth, or specific achievements.
You could even add adapted goals based on Common Core Standards to a format like this one so instead of assessing only IEP objectives, those taking data could also note how the student is progressing in the general education curriculum. For instance, any content area vocabulary words mastered that week might be added to the social studies section.
